Form templates
Ask ordered questions and turn answers into an Issue description or real Issue properties.
Build a form
Open the workspace or Team template settings, select New template, and choose Form. You need the same management access as for Standard templates.
Give the template a name and optional description. In the Issue card, use Starting title (optional) and Starting description (optional) for content the Issue starts with before answers are added. The card also holds defaults and sub-issues.
Under Questions, open Add question and choose a type. The menu groups ordinary Questions separately from Issue properties. You can add up to 30 questions, including Instructions. A new question starts without a name: write the real question, such as "What happened?", in Question, or a Heading for Instructions. The template cannot be saved while a question has no name; the question is marked until you name it. Issue property questions start with the property's name, which you can change. Reorder questions with the drag handle or the ⋯ menu's Move up and Move down actions. Use Remove question in that menu to remove one.
Choose question types
| Question | What it collects |
|---|---|
| Short text | A short answer, up to 500 characters |
| Long text | A longer answer, up to 5,000 characters |
| Dropdown | One option from a list |
| Checkboxes | Several options from a list |
| Date | A calendar date |
| Instructions | Guidance shown in the form; no answer needed |
For dropdowns and checkboxes, enter one option per line, with up to 50 unique options. Turn on the Required switch for questions that need an answer. Instructions cannot be required.
A Date answer becomes description text. Use a Due date question when you want to set the Issue's actual deadline.
Set real Issue properties
In Add question → Issue properties, choose Title, Priority, Labels, or Due date. Their answers set the real Issue property instead of adding text to the description. Each question has a tag such as Sets Issue title or Sets Issue priority. A property already used in the form shows Added in the menu and cannot be added again.
For a Labels question, choose its Label group. The creator can then choose from that group's available labels. The question's tag says Sets Issue labels; a Due date question says Sets Issue due date.
Make a property question required when it must be answered. No priority does not satisfy a required Priority question. An Issue always needs a title, even if the form has no Title question. Property answers and the ordinary Issue property controls edit the same values.
Under Defaults, set starting properties that creators can change. The editor notes: People outside the Team don't get defaults. When the form has a Priority question, the Priority control becomes Priority · set by question. Saving removes any stored Priority default, so the answer supplies the priority. Removing that question later does not restore the old saved default.
Try the preview
Preview has two parts: What the submitter fills in lets you enter sample answers; The Issue it creates shows the resulting title, properties, and description. Each answered question appears in the description as a bold line with its question text or heading, with the answer below it, in question order. Dates read like "Oct 13, 2026", checkbox answers appear as a list, and questions left empty are left out. Property answers appear as Issue properties instead of description text.
The note Sample answers aren't saved means these answers only test the preview; they are not default responses. Select Create or Save to save the template definition.
Fill in a form
Choose the form in the New issue dialog and answer its questions. Required questions are marked, and Create stays disabled until they are answered. Review the title and any property answers before submitting.
The resulting Issue has the composed description, which you can edit like any Issue description. The answers as they were sent stay in the Issue's read-only Form submission section, below the description. It starts collapsed and shows who submitted the form and the date. Open it to see the submitter and time, the answers that set Issue properties (such as Priority or Labels) shown like the Issue's own properties, and each written question with its answer below it. A Title answer appears only if the Issue has been renamed since. Editing the description does not change it. Very large forms can exceed the Issue description limit of 200,000 plain-text characters; shorten the answers if creation is refused.
The Issue's Activity shows Submitted with the form name form, with View answers to open the Form submission section. Anyone who can view the Issue can read the answers, because they are part of the Issue. Only people who can see the template see its name: everyone else sees Submitted with a form and a Form submission section without the template name.
Forms can be selected manually by members or non-members, but only the non-member Team default can be a Form. For non-member submissions, the Team's existing Triage rules still apply.
